City Driver’s Authorization Information Form

This form will allow you to provide Risk Management the necessary information in order for an employee of the City of Corpus Christi to be eligible to operate any vehicle on City business.

Please submit this form ONLY when the employee is newly hired/transferred and during the renewal of their driver’s license.
